As of 24 January, 2022 ERASA will be supplying passports for Endurance horses only

FULL date of birth and microchip number to be supplied

FORM J PASSPORT AND UV REGISTRATION FORM J	This form is to be completed for passport and horse registration with ERASA - endurance horses only and submitted with Horse ID Centre page and proof of payment to ERASA - THIS FORM TO BE SUBMITTED WITH HORSE ID CENTRE PAGE
HORSE ID CENTRE PAGE	This form to be completed by and stamped by a registered veteranarian and submitted with Form J - this Form then to be scanned in colour and submitted
FORM J (B) - Horse registration only	This form is to be used if the horse already has a passport. Copies of description page to be submitted in colour
FORM J (a) UV REGISTRATION FOR 2 X 40KM ONLY FORM J (c) NEW PASSPORT ONLY	This form is to be used for the registration of a horse without a passport.This will allow the horse to enter 2 x 40km only, whereafter a passport must be obtained. The horse must be microchipped and be 5 years or older. This form is to be used when horse has a UV Number and only needs a passport.
RE ISSUE PASSPORT HORSE ID CENTRE PAGE	This form is to be used only if the passport has been lost or the passport full , horse id centre page to be completed and submitted with application for re-issue passport
